1) When James first began crime fighting, he used his white doctor’s uniform, complete with facemask. He often covered it with a blue trenchcoat which concealed an array of hypodermic guns. [Lightning Comics (1st series) #6]
2) After forming the Battle Axis team and changing his name to Doctor Death, James altered his look slightly by wearing more formal white clothes underneath a white trenchcoat. He kept his surgeons mask but changed his surgeons cap for a white fedora. [The Invaders (2nd series) #1]
3) After decades of hunting down Nazis James joined the X-Club. Keeping with his signature look, he now wore a formal shirt, trousers and tie under a white labcoat. He continued to wear his fedora and surgeons mask on missions. [Uncanny X-Men (1st series) #512]
4) As part of X-Force, Nemesis kept the general aesthetics of his regular look, but turned it into a more protective version due to the dangerous nature of the team’s missions. Underneath his labcoat he wore white body armour, complete with numerous belts and pouches to hold the ammunition for his hypodermic guns. His facemask now resembled a gasmask, complete with air filtration devices on it, and he often wore goggles. Later on his altered the colour of his boots and gloves from white to orange. [Cable and X-Force #1]
-) As part of the next iteration of X-Force, Nemesis went back to his X-Club look and has kept that look ever since
