When Professor Xavier resettled to Genosha in order to help rebuild the island nation, he left the Institute in the capable hands of Scott Summers and Emma Frost. They re-organized the X-Men teams, assigning the various X-Men to one of the three groups. For Cain, though, only little changed, as he remained on the group that was led by Havok. Some of the others members, especially Iceman and Wolverine, still felt that, as a former villain, the Juggernaut had no right to be on the team and expressed their disapproval of him at every turn. However, compared to the personal problems and relationship issues of his teammates, the Juggernaut seemed to be the one member of this group who had his act most together. This distrust of Cain seemed to prove correct, for soon after an all-new Brotherhood made their debut, including Juggernaut’s old friend, Black Tom. Apparently, they had long planned for Cain to gain the X-Men’s trust and infiltrate the mansion, and Black Tom had been hiding in the woods of the Xavier estate for the past several weeks. In the meantime, however, Cain had changed his mind and was genuinely attempting at redemption. Juggernaut secretly alerted his team leader Havok to the situation and only faked at playing along the Brotherhood’s plans. Unfortunately, the villains' meeting was witnessed by young Sammy, who was devastated to see his idol in league with the bad guys. Sammy confronted Cain and threatened to alert the X-Men, only for Black Tom to kill the boy quicker than Cain could react. Sammy died thinking the worst of Cain, and Cain was horrified that he would never get the chance to explain his actions. [X-Men (2nd series) #161-162]

Outraged by his young friend’s death, Cain openly rebelled against Tom and the Brotherhood, though they still almost succeeded in destroying the mansion and killing everyone present. In the end, the Brotherhood were defeated when Nocturne possessed Black Tom long enough for Xorn to suck the Brotherhood inside his black hole mind. However, it came with a cost. Nocturne was still inside Black Tom, and Juggernaut also volunteered to be sucked into the black hole, so that he could look after Nocturne, keep the Brotherhood in check if needed and hopefully redeem himself to the deceased Sammy. Somehow, the party ended up tossed through dimensions until they wound up on Mojoworld. The Brotherhood quickly negotiated their freedom with Mojo by selling Juggernaut and Nocturne into his service. After a few weeks, Nocturne managed to secure their release by possessing Spiral, and using her to teleport them back to Earth-616 at the mansion. Unfortunately, Mojo and Spiral had planned for this and Mojo himself followed them through to play with his favorite entertainers in the X-Men. He and Spiral used a "Jean Bomb" to reduce the X-Men into X-Babies, back by popular demand. Despite their reduced size and maturity, Juggernaut and the other X-Men were able to capture and defeat the invaders. Baby Juggie initially felt like remaining a child. Still grieving over Sammy's death, Cain thought he could get a "do over" and relive his life in a manner Sammy would be proud of. However, while Mojo tempted Cain with his own desires, Storm ultimately convinced him to own up to his past and accept adulthood and the responsibilities that came with it. [Uncanny X-Men (1st series) #460-461]

After the House of M reality swap, Juggernaut and Nocturne accompanied several X-Men to England where they compared vague memories with Captain Britain about their time in that other world. While in Britain, they were alerted by Pete Wisdom of MI-13 to an attack on the former X-Man, Dazzler. A huge fan of Dazzler's music from way back, Cain was particularly insistent in checking on her. In the confrontation with a corrupted, alternate dimensional “Shadow X-Men” that followed, however, Juggernaut "terrorized" the city of London while chasing the villains. [New Excalibur #1-3] Cain was placed on trial as a "menace to society" but Wisdom used MI-13's connections with the Crown to make the charges go away. In exchange, Wisdom recruited Juggernaut and Nocturne as part of England's new team of heroes, Excalibur. [New Excalibur #4-5]

When Excalibur ran inference for MI-13 during the transfer of their Shadow X-Men prisoners, the group was attacked by Black Air. Black Tom Cassidy (fresh from being at least partially depowered on M-Day) had hired out as a mercenary to Black Air on the mission. Tom ended up on the run after Black Air acquired Shadow-X and abandoned him, so he broke into a house and desperately took a woman hostage. Cain went in after Tom and confronted him about Sammy's death. Tom heatedly denied responsibility for the boy's death, claiming he wasn't in his right mind at the time. Juggernaut admitted he would deeply love for Tom to resist or hurt the woman, giving him an excuse to enact some revenge for Sammy, but because of their history together he was willing to give Tom a chance to do the honorable thing and surrender for his crimes. Tom soon relented and Juggernaut took him into custody without hurting him. [New Excalibur #6-7]

His encounter with Black Tom wasn't the end of Cain's troubles, however. He found himself in the middle of an awkward love triangle between Nocturne and Dazzler. TJ started thinking about Cain as more than a friend, although he didn't reciprocate. Instead, his idol worship of Alison had Cain dreaming of a relationship with her. For her part, Alison wasn't interested in Cain romantically, but his affection was the only fame and adoration she was getting in her music career at the moment, and so she did little to discourage his crush.

Juggernaut was also suffering from an ongoing and increasing weakness in his powers. His heroic attitude was taking him further and further from what Cyttorak wanted in an avatar. After a particularly humiliating fight with his former drinking buddies the Wrecking Crew, Cain was determined to reclaim his former glory. He flew to Korea to approach Cyttorak at his temple but Cyttorak was ready to abandon his avatar, calling him weak and pitiful. To Juggernaut's shock, Cyttorak revealed that Charles Xavier was his first choice to become his avatar all those years ago, and he only accepted Cain as a consolation prize. Cyttorak offered Cain the chance to defend his title against a new Juggernaut recruited out of Seoul. Cain fought back to prove his worthiness and was on the verge of killing his replacement for Cyttorak when Excalibur arrived looking for him. Their arrival snapped him back to his senses and Cain did not kill the new avatar. Even after Cyttorak showed Excalibur some of Juggernaut's atrocities in his name, such as the massacre of Tenko's village, they ultimately took him back. For his part, Cain did not renew his vow to Cyttorak, but he did secretly keep the ruby gem upon returning to London, just in case. [New Excalibur #13-15]

In the weeks that followed, Cain seemed more well-adjusted and spent time helping TJ recover from a stroke, as well as defending England from the invasions of Albion and his Shadow Captains. [New Excalibur #16-24] The temptation to use his power rose once again, however, when the Hulk invaded Earth with an alien army. Hulk attacked the X-Mansion to get at Xavier, who had been part of the group called the Illuminati that voted to exile Hulk to space in the first place. The X-Men telepathically called for help from their various splinter groups, but Cain and Excalibur were too far away in England to help in time. Cain recovered the ruby from his room and offered to serve Cyttorak fully again if he would transport him to Westchester to save his brother. Cyttorak did send Cain through the Crimson Cosmos to the mansion but, when Juggernaut leapt into battle with the Hulk, he realized he was just as weak as ever. When he demanded to know why, Cyttorak told Cain his reasons for fighting the Hulk were weak -- Cain didn't really care about protecting a brother he had hated for so long. With Cyttorak's urging, Juggernaut admitted he wanted to beat on the Hulk for his own sake, to crush a powerful opponent, to fight, to win. With this as his driving force, Cain Marko once again became a true, human Juggernaut. He returned to the battle and battered the Hulk mercilessly, holding his own against the rage-engorged Hulk like no one else. In the aftermath of the battle, however, Juggernaut left Excalibur and the X-Men behind. Recommitted to Cyttorak's cause of conflict and devastation, the lingering remnants of his heroism urged Cain to get as far from his friends as possible. [World War Hulk: X-Men #2-3]

Over the next several months, Juggernaut struggled to balance his renewed taste for power with his recent efforts at redemption. When the X-Men disbanded and then relocated to San Francisco, Cain considered joining them, and took a rest stop bar hostage for several hours as he encouraged his captives to weigh in on whether he should be "good" or "bad." He actually decided to be a good guy but, when the police shot at him when he exited the bar, Cain fell back into his destructive ways. [X-Men: Manifest Destiny #2] He was provoked into a fight with Skaar, Son of Hulk, by Bruce Banner as a combat lesson, [Incredible Hulk (2nd series) #601-602] and resumed his murderous feud with his step-brother, Charles Xavier. [X-Men: Legacy (1st series) #219]

Juggernaut's past came back to haunt him when a man named William Nguyen received the Uni-Power, and became the latest cosmically-powered hero known as Captain Universe. Nguyen blamed Juggernaut for ruining his life after he lost his job due to one of Cain's rampages years earlier, and used his newfound power to seek revenge on Juggernaut. The Uni-Power chooses hosts for a particular mission, however, and it soon became clear what William Nguyen's was. When Cain forced his way out of the cement trap in which Spider-Man left him in years back, he charged his way through Manhattan's bedrock, leaving scars that made the entire island seismically unstable. When Nguyen refused to put aside his vendetta against Marko to attend to his appointed task, the Uni-Power abandoned him and passed into Juggernaut. As the new Captain Universe, Juggernaut stopped the earthquakes, reversing the damage he did to the tectonic plates and saved the entire city. [Amazing Spider-Man (1st series) #627-629]

This heroism wasn't without a price, however. Cyttorak was quite unhappy with his avatar serving another force, particularly in the execution of heroic acts that specifically undid devastation previously done in his name. As a result, Juggernaut once again began to weaken and ended up being captured and imprisoned aboard the Raft. Luke Cage was heading up a new Thunderbolts work release program for inmates at the prison and, with the secret endorsement of Charles Xavier, he accepted Juggernaut into the program. Marko naturally tried to run the first chance he got, but his weakened powers left him at a disadvantage on the island prison — he could no longer survive without breathing, nor could he swim to freedom with his enormous mass. After confirming his current power levels, the Federal Advisory Committee for the Thunderbolts (FACT) outfitted him with inhibiting nanites and cleared him for duty. Once he established that he couldn't escape, Juggernaut began to welcome the privileges of being a Thunderbolt. A Juggernaut needs to move around to feel at ease, and the constant confinement was pathologically uncomfortable for Marko. He proved to be brutal but effective on the Thunderbolts' early missions on behalf of the government. When Ghost overheard Luke Cage planning to leave the Thunderbolts and possibly shut the program down, however, Juggernaut tried to escape again out of fear of being caged full time once more. He only calmed down when Cage agreed the program had its uses and agreed to continue supporting it. [Thunderbolts (1st series) #150]

Juggernaut established some camaraderie with his fellow Bolts, Moonstone and Ghost, and respect for the team. Their team unity was clear when the powerful but unruly Hyperion was added to the roster and tried to break off from the group. Juggernaut and the Thunderbolts curb-stomped the unpleasant villain when he turned on them, and drummed him out of the program. [Thunderbolts (1st series) #152-153] The Thunderbolts program did expand, however, with a B-team of new draftees that Moonstone nicknamed the "Underbolts." Feeling like FACT was testing out replacements for him, Cain's old insecurities started to rise to the surface. When he and Luke Cage faced a fear monster in a Nazi ghost castle, Juggernaut hallucinated his teammates and superiors declaring him inadequate and locking him away again. His anger continued even after the hallucination ended, and he assaulted Cage on the mission. Luke, already shaken from hallucinating the death of his daughter, overreacted and benched Juggernaut indefinitely when the Thunderbolts and Underbolts were called out on another mission, deliberately provoking Cain's fear of being replaced.
